Naval Forms Online is the Department of the Navy's repository for all Navy and Marine Corps officially approved forms.
The DON Social Security number (SSN) reduction plan for forms (phase one) requires a review of all forms that collect SSNs. If continued collection is required, a justification memo must be on file with the applicable command's forms manager (i.e., the command that owns the form) signed by a flag officer, senior executive service employee or individual with by direction authority. If the form is no longer required, it must be cancelled. If continued collection of SSNs is not required, the form must be updated by removing the SSN or substituting it with another unique identifier.
