PII Breach Reporting Forms

These two forms are available for use in accordance with DTG 291652Z FEB 08: Loss of Personally Identifiable Information Reporting Process.
OPNAV 5211/13:DON Loss or Compromise of Personally Identifiable Information (PII) Breach Reporting Form is used for initial and supplemental breach reporting. OPNAV 5211/14:DON Loss or Compromise of Personally Identifiable Information (PII) After Action Reporting Form is for after-action breach reporting. Email buttons are provided with built-in distribution lists. When an email button is selected, an email window will open with the form as an attachment. Here you can add additional addressees and text. The subject line should indicate "PII Breach Report for [Command Name - US-CERT number if available]."
